Current Classification of Midshaft Clavicle Fractures

This study investigates multiple fracture patterns in midshaft clavicle fractures using 3D computed tomography (CT) instead of traditional radiography-based classifications.

Key Findings:

Three distinct fracture patterns were identified:
Type 1: Posteromedial to anterolateral fracture line located between 50% and 68% of the clavicle length.
Type 2: Transverse fracture line around 55%.
Type 3: Superolateral to inferomedial fracture line between 47% and 56%.
Wedged fracture fragments are commonly observed in Types 1 and 2, primarily located inferiorly.
Displacement direction varies depending on the fracture pattern (posterior, anterior, inferior).

Methodology:

The study analyzed CT scans of 65 acute clavicle fractures using:

3D reconstruction: Fractures were virtually reduced for detailed analysis.
Quantitative 3D CT techniques: Heat maps and surface curve analysis determined fracture line distribution and fragment location.
