Summary

Guidances suggest waiting until symptoms subside before immunizing individuals with severe acute illness. For those with minor symptoms, the COVID-19 Self-Assessment Tool can be used to determine if influenza immunization can be provided. Typically, people only get one virus at a time, and post-viral CAP or concurrent CAP is uncommon. There isn't yet an RSV vaccine, but RSV prophylaxis is available for high-risk infants. Influenza typing has not been done yet to determine if there's a good match to the vaccine this year. Strategies for managing volumes in after-hours clinics include triaging patients and providing education on managing symptoms at home. The supply of masks in community practices is sufficient, and costs are lower compared to the early days of the pandemic. Gowns are probably less useful compared to eye protection and masking for screen-positive patients. For COVID-19 testing and treatment, patients who are high-risk can access PCR testing, and Paxlovid is only available if a patient tests positive and is within 5 days of the test/symptoms. Evusheld is available for prevention in very high-risk groups. Eligibility criteria for Paxlovid may differ from Science Table guidelines, and treatment should be considered based on individual circumstances. Both Paxlovid and remdesivir are not recommended for patients with eGFR <30. If a RAT test remains positive for several days after a COVID infection, it does not necessarily mean the patient is infectious, and they can be considered non-infectious if symptom-free.
