Summary

The US healthcare cost has skyrocketed over the past half-century, with total national health expenditures exceeding $3.7 trillion in 2018. Out-of-pocket consumer costs have also increased, with the average general annual deductible for single coverage being 162% higher in 2019 compared to 2009. Cost-sharing deters patients from receiving care, including high-value preventive and chronic disease management services. The Patient Protection and Affordable Care Act (ACA) eliminated out-of-pocket costs for high-value preventive care services among private, non-grandfathered health insurance plans. A rapid review of the literature found that the impact of cost-sharing elimination on utilization of preventive services varied depending on the clinical service, with a majority of findings showing increases in use. Studies reported substantial increases in utilization among financially vulnerable populations. Future investigations on additional clinical services are warranted to better elucidate populations who most benefit from cost-sharing elimination.
