The International Union of Operating Engineers Local 825 Welfare Fund Notice of Privacy Practices describes how medical information about individuals may be used and disclosed, and how they can access this information. The Fund is required by law to ensure the privacy of personally identifiable health information and inform individuals about its use and disclosure. Protected Health Information (PHI) includes health information that can identify an individual, such as name, address, and Social Security number. The Fund may disclose PHI without consent in certain cases, including upon request, as required by government agencies, and for treatment, payment, and health care operations. The Fund may also use or disclose PHI for other purposes, such as reminders, treatment alternatives, health-related benefits and services, and disclosure to the plan sponsor. Individuals have the right to file a complaint with the Fund and the Secretary of the United States Department of Health and Human Services. The Fund reserves the right to change its privacy practices and will provide a revised Notice of Privacy Practices if material changes are made.
