Summary

Patient will wear an abduction pillow sling for 6 weeks postoperatively and not begin physical therapy until 2-week postoperative visit. The rehabilitation protocol consists of 5 phases:
Phase 1 (0-4 weeks): Maximal protection, elbow A/AAROM, protect anterior and posterior capsule, no pendulums, modalities PRN, wrist and gripping exercises.
Phase 2 (4-6 weeks): Moderate protection, progress shoulder PROM, glenohumeral joint mobilizations, address scapulothoracic and truck mobility limitations, introduce posterior capsular stretching.
Phase 3 (6-12 weeks): Minimal protection/mild strengthening, progress PROM, AROM activities, quality movement only, avoid forcing active motion with substitution patterns.
Phase 4 (12-16 weeks): Strengthening, pain-free AROM, progress by response to treatment, restore scapulohumeral rhythm, joint mobilization, aggressive scapular stabilization and eccentric strengthening program.
Phase 5 (16-20 weeks): Overhead activities/return to activities phase, avoid excessive anterior joint capsule stress, continue prior phase exercises, overhead strengthening, return to upper extremity weightlifting program.
