This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Hazelden Betty Ford Foundation and Recovery Partners, P.C. are required by law to maintain the privacy and confidentiality of your health information. Protected health information is protected by 42 CFR Part 2 and the Health Insurance Portability and Accountability Act (HIPAA) Privacy Regulations.

Generally, your authorization is required for use or disclosure of your protected health information, but there are exceptions, such as for treatment, health care operations, appointment reminders, medical emergencies, and reporting of child abuse. Your authorization can be revoked, except to the extent that action has already been taken.

Uses and disclosures without your authorization include treatment, health care operations, and other allowable uses, such as disclosure to medical personnel in a medical emergency, to a parent or guardian of a minor, or to a coroner or medical examiner. Your protected health information may also be disclosed in response to a court order, or if you commit a crime on the premises or against program personnel.
