A perineal tear is a tear to the perineum, the area between the vagina and anus, occurring during childbirth, with 85 out of 100 women experiencing one after a vaginal birth. Tears are graded by severity: labial tears, 1st degree (skin), 2nd degree (skin and muscle), 3rd degree (skin, muscle, and anal muscle), and 4th degree (skin, muscle, anal muscle, and anal canal lining). Risk factors include first vaginal birth, unusual baby position, quick birth, forceps use, large baby, and prolonged second stage of labor. Diagnosis is by examination after birth, and treatment varies by tear severity, with labial and 1st degree tears often healing naturally, 2nd degree tears stitched with local anaesthetic, and 3rd and 4th degree tears repaired in theatre with anaesthetic. To reduce risk, women can try giving birth in water, kneeling or in an 'all fours' position, massaging the perineum, and eating a balanced diet. After a tear, women can manage pain with painkillers, keep the area clean, and avoid constipation to promote healing.
