Summary

Cataract remains the leading cause of blindness in middle- and low-income countries, contributing to approximately half of all causes of blindness in Africa. In Zambia, eye conditions are a major cause of hospital admissions and outpatient visits, with a blindness prevalence of 1.1% in the general population and 2.8% in people aged 50 and above. A study was conducted to understand demand for cataract surgery and investigate factors influencing its uptake in the context of the Coordinated Approach to Community Healthy Eyes (CATCH) program. The study found that out of 517 patients screened and referred for surgery, only 25% (127) had available surgery records, with a possible uptake rate of 68% if all 353 operated patients were CATCH referrals. Qualitative data identified individual, community, and program/health system factors influencing patients' decisions, including experiences of previously operated patients, cultural beliefs, misinformation, and rumors.
