Summary

Spirometry underutilization persists among patients with COPD in urban tertiary care centers, despite reduced access barriers. Factors associated with underutilization include older age, not having received pulmonary care, and having fewer comorbidities. Only 59.7% of 24,300 patients with a new COPD diagnosis between 2012 and 2015 underwent spirometry within 2 years before or after diagnosis. Patients 75-84 years old had an adjusted odds ratio of 0.80, and those ≥85 years old had an adjusted odds ratio of 0.47 for undergoing spirometry, compared to patients <55 years. Patients with a Charlson Comorbidity Index of 0 had an adjusted odds ratio of 0.60, and those who had not seen a pulmonary specialist had an adjusted odds ratio of 0.38 for receiving spirometry.
