1. Students applying for autonomous learning should fill out this form, obtain the guiding teacher's agreement signature, and submit it to the "Library" for review along with relevant planning materials before the administrative unit's deadline.
2. During autonomous learning, students who need to leave the classroom must wear their ID badges for identity verification and course confirmation.
3. The autonomous learning plan should be written and signed by the student, mentor, and parent within the first three weeks, and stored in the student office for weekly review to track the student's learning content and progress.
4. The final outcome report (with examples on the library website) procedure: The guiding teacher will submit the autonomous learning plan to the library for review, and then the student will upload it.
