The Danish healthcare system is built around different sectors, causing patients' courses of disease to be less coherent than ideal. This thesis investigates the strengths and weaknesses of three initiatives to solve this problem: video discharge interviews, a cross-sectoral network between health professionals, and municipal discharge officers. The study concludes that when initiatives assist parties in creating a positive dialogue and relations across sectors, it is a clear strength, helping different parties understand each other and their differences. The importance of dialogue, well-organized background, and specialized staff is emphasized. Involving patients and their relatives in the cross-sectorial effort is also a strength. Weaknesses include the absence of positive dialogue and necessary knowledge among health professionals. The study highlights the importance of health professionals sharing the same goals and notes that all initiatives struggle with practical and economic challenges. The conclusion is based on an analysis of eight qualitative interviews.
