Summary

Barking and Dagenham applied to the Design in Social Care programme in 2018 to move towards a more person-centred and asset-based approach in social care. The project aimed to understand how people could be more involved in their care and support planning, particularly when leaving hospital. Semi-structured interviews with people in receipt of care and carers revealed themes such as financial concerns, confusion about what happens when leaving hospital, and the need for coordination between organisations. The team developed a personalised guide to inform people about their care and support after leaving hospital, and plan to further develop and test it with patients, carers, and healthcare professionals. The long-term goal is to strengthen communication between local partners and continue to involve people in their care and support planning.
