It consists of two main parts: a personal details form (Form 33) and an intimation of change of address form.

Form 33 (Personal Details):
- Applicant's personal data (name, gender, date of birth, age, place of birth, parents' names, region, addresses, PIN codes, sub-districts, districts, states, village names, marital status, spouse's name, email ID, contact phone number, voter ID card number, Aadhaar number, and Aadhaar enrollment number if Aadhaar not issued).

Intimation of Change of Address Form:
- The registered owner of a motor vehicle notifies the Registering Authority about the change of address.
- If the vehicle is held under an agreement of hire-purchase, lease, or hypothecation, the form must be made in triplicate, with copies returned to the Financier and the Registering Authority.
- The form includes the vehicle's details, the old and new addresses, and a request to record the change in the certificate of registration and Form 24.
- If the vehicle is held under an agreement, the Financier's consent is required, and their signature is needed on the form.
- The Registering Authority endorses the form, enters the change of address, and sends copies to the Registered Owner and the Financier.

Consent of the Financier:
- The Financier gives consent for the change of address or refuses it, stating reasons if refused.

Specimen Signatures:
- Specimen signatures or thumb impressions of the Registered Owner and Financier are required for attestation by the Registering Authority.
