It consists of three parts:

1. Personal Details (Mandatory fields marked with *)
- Application number and date
- Name, father's name, mother's name, date of birth, gender, marital status, spouse's name (if applicable), contact number, and email ID
- Region, permanent address, PIN code, sub-district, district, state, and village (if rural)
- Correspondence address (same as or different from permanent address)

2. Service Details
- License number and category (Individual, Sports Person, Member of Rifle Club/Association, Company/Bank)
- Address of the branch or representative office
- License validity date and area validity (District, State, or All India)
- Nearest police station
- Existing weapon details (up to 10 weapons, depending on license category) and details of weapons whose bore is to be changed (up to 3 weapons)

3. List of Required Documents
- Identification cum residence proof (Aadhar Card, Passport, Voter's ID, Driving License, or Valid Govt. ID Card)
- Copy of Arms License
- Any other attachment (optional)

Declaration:
- The applicant solemnly declares that the information provided is correct and no information has been hidden. They are aware that if any information is proved incorrect, they can be punished and all government benefits may be stopped.

Note:
- For Junior Target Shooters under 18, an adult family member must declare.
- The form is in English and Punjabi.
