Summary

To obtain Critical Care or Chronic Condition Status with a utility, customers must complete an application, which will not be processed if incomplete, unreadable, or improperly submitted. The application requires customer and patient information, including name, address, and contact details, as well as a physician's certification of the patient's medical condition. The physician must complete Page 3 of the application and submit it to the utility via fax or email. Submission of the application does not automatically result in chronic condition or critical care status, and notification of the status granted will be provided to the customer. Designation as a chronic condition or critical care residential customer does not relieve the customer of the obligation to pay for electric service, and service may be disconnected for failure to pay. Chronic condition or critical care status does not guarantee an uninterrupted power supply, and customers must make other arrangements for backup capabilities in the event of a loss of electric service.
