Summary

To qualify for Chronic Homelessness Status, a person must have a disability expected to be long-lasting, substantially impeding independent living, and could be improved by suitable housing. Qualifying disabilities include Mental Health Disorder, Substance Use Disorder, HIV/AIDS, Physical Disability, and Developmental Disability. Verification of disability requires third-party documentation, such as written verification from the Social Security Administration or a disability check. In the absence of these, an agency staff person's observation of a qualifying disability may be used temporarily, but must be confirmed within 45 days. For HUD-funded projects, failure to secure verification within 45 days may impact funding. Chronic homelessness is defined as being continuously homeless for 12 months or having at least 4 occasions of homelessness over 3 years, totaling 12 months, with breaks of at least 7 consecutive days between episodes.
