Summary

A fatality occurred on a floating MODU when two employees were disconnecting a "U" tube from an isolation valve to repair a damaged marine riser tensioner. The internal parts of the valve blew out, striking an employee, due to a misidentified flange and failure to vent 400 psi of air on the 3-inch line. Causes included lack of knowledge about the valve assembly, a false sense of security, and pressure left on the line. Corrective actions included instructing personnel to vent pressure before work and tagging out isolating valves, and planning to issue further recommendations.
