- Based on income, ranges from 1% to 3% of medical fees exceeding the limit
- Limit per Month (per household): 252,600 yen (active income earners), 167,400 yen (income 770-1,160 million yen), 80,100 yen (income 370-770 million yen), 57,600 yen (general), 18,000 yen (low-income), 15,000 yen (lowest-income)
- Limit per Month (per individual): 140,100 yen (active income earners), 93,000 yen (income 770-1,160 million yen), 44,400 yen (income 370-770 million yen), 44,400 yen (general), 24,600 yen (low-income), 14,400 yen (lowest-income)
- High-cost Medical Fee Reimbursement: Excess over the limit is reimbursed by the insurer

- High-cost Medical Fee System Overview:
- Example: 70 years and above, income 370-770 million yen, medical fee 100 million yen
- Window co-payment: 30 million yen
- Co-payment limit: 87,430 yen
- Reimbursement: 212,570 yen

- 70-74 years: 2% (18,000 yen limit)
