The Medicaid Spend-down Program allows individuals with excess income to qualify for Medicaid by paying or incurring medical costs equal to or exceeding the excess amount. Eligibility is limited to those 65 or older, certified blind or disabled, under 21, or parents/caretaker relatives of children under 21, with no excess resources. The excess income is calculated by subtracting the monthly Medicaid income limit from countable income. There are two ways to spend down excess income: through the Medical Bills system, where individuals submit paid or unpaid medical bills, or the Pay-in Program, where individuals pay the excess amount in advance. Recertification is required every 12 months.
