## One-Parent Family Payment (OFP) Application Form Summary

This form is for individuals applying for OFP, a payment for parents raising children without a partner aged under 66.

Eligibility:

Under 66 years old.
Parent, step-parent, adoptive parent or legal guardian of a relevant child (under 7).
Have primary care and charge of at least one relevant child.
Meet a means test (maintenance received for a child excluded).
Not living with a spouse, civil partner, or cohabiting partner (exceptions apply for separated/divorced individuals).

When to Apply:

Widowed or surviving civil partner: Within 3 months of partner's death.
Single: Within 3 months of child's birth.
Separated, divorced, or no longer in a civil partnership: Within 3-6 months of separation.
Prisoner’s spouse/civil partner: When incarcerated for at least 6 months without sentence or upon start of a sentence of at least 6 months.

How to Complete:

Use black ballpoint pen, capital letters, and check relevant boxes.
Answer all applicable questions.
Fill in Parts 1-6 and 9A; employer should complete Part 9B if applicable.
Sign the declaration in Part 7 after reviewing the checklist in Part 8.

Supporting Documentation:

Payslips (3 recent) for employed individuals.
Certified accounts for self-employed individuals.
Proof of mortgage payments (if homeowner).
Lease/tenancy agreement and recent receipt from landlord (if renter).
Copy of Maintenance Order or Separation Agreement (if applicable).
