The Occupational Safety and Health Administration (OSHA) issued a Safety and Health Information Bulletin (SHIB) to inform employees and employers about the hazards of orthostatic intolerance and suspension trauma when using fall arrest systems. Orthostatic intolerance occurs when blood accumulates in the veins, reducing blood flow to the brain, and can cause symptoms such as lightheadedness, palpitations, and fainting. Suspension trauma, also known as "harness-induced pathology," can occur when a worker is suspended in a harness for an extended period, leading to unconsciousness and potentially death. Factors that can increase the risk of suspension trauma include inability to move legs, pain, injuries during the fall, fatigue, dehydration, hypothermia, shock, cardiovascular disease, respiratory disease, and blood loss. OSHA recommends rescuing suspended workers as quickly as possible, being aware of signs and symptoms of orthostatic intolerance, and providing training on the use of fall arrest systems and other personal protective equipment. Employers should implement plans to prevent prolonged suspension in fall protection devices, including procedures for preventing prolonged suspension, identifying orthostatic intolerance signs and symptoms, and performing rescue and treatment as quickly as possible.
