Summary

Das Formblatt für die Abschätzung der Radonexposition der Arbeitskräfte pro Kalenderjahr an angemeldeten Arbeitsplätzen mit erhöhter Radonkonzentration besteht aus drei Abschnitten:

I. Allgemeine Angaben: Enthält Felder für die Angabe des Betriebsnamens, der Adresse, des Verantwortlichen für die Arbeitsplätze sowie des Ansprechpartners für den Strahlenschutz vor Ort.

II. Bemerkungen: Ein Feld für eventuelle Bemerkungen oder zusätzliche Informationen.

III.1 und III.2: Enthält Felder für die Angabe der Anzahl der Arbeitskräfte und der Anzahl der Arbeitskräfte mit einer effektiven Dosis größer 6 Millisievert im Kalenderjahr.

Das Formblatt muss von einer verantwortlichen Person unterschrieben und datiert werden, um die Richtigkeit und Vollständigkeit der Angaben zu bestätigen. Das ausgefüllte Formblatt ist an das zuständige Regierungspräsidium zu senden.
