Subcontract Checklist.pdf

1. Verify project number and Federal Aid Project (FAP) on subcontract.
2. Check contract for:
a. DBE items
b. Cumulative percentage of approved subcontracts
c. Compare subcontract items to contract items and compute percentage at contract bid price (cannot exceed 70%).
3. Requirements:
a. Request letter from Prime Contractor for review and acknowledgment of proposed subcontract.
b. Subcontractor must have current Contractor’s License for Arkansas if subcontract is $50,000 or more.
c. Federal-Aid Projects:
- EEO Certification form for subcontracts $10,000 or more.
- Certification for Federal-Aid Contracts for subcontracts $100,000 or more.
d. Subcontracts must state payment within 10 days of Prime receiving payment from Department.
e. Subcontracts must include Title VI statement.
f. SWPPP certification form must be included in each subcontract if contract has SWPPP special provision.
g. EACH subcontract on Federal-Aid projects must include Form FHWA-1273.
h. Check for statements contrary to specification.
i. Note Partial Work subcontracts and include description.
j. List pay items as they appear in contract with units of measure.
k. Subcontract must be signed by both parties.
l. DBE Purchase Orders must be sent to ARDOT Construction Division for review before work begins.
