Approximately 13 million people in the US are eligible for both Medicare and Medicaid, with significant health and social service needs. Integrating financing and delivery systems for this population can improve care experience, quality, and reduce costs. As of 2022, 21% of full-benefit dually eligible individuals were enrolled in integrated programs, including demonstrations under the Financial Alignment Initiative (FAI), Integrated Dual Eligible Special Needs Plans (D-SNPs), and Programs of All-inclusive Care for the Elderly (PACE). Nine states are pursuing FAI demonstrations, with total enrollment of approximately 300,000 in January 2023. D-SNPs are available in 45 states, with 12 states having fully integrated D-SNPs (FIDE SNPs) and 15 states having highly integrated D-SNPs (HIDE SNPs), totaling 4,888,866 enrollees as of January 2023. PACE organizations operate in 32 states, enrolling 55,765 individuals. States are advancing alignment of Medicare and Medicaid services for dually eligible beneficiaries, with federal support, and will continue to refine opportunities and develop alternative pathways.
