Summary

A 55-year-old woman with primary hypothyroidism, taking levothyroxine for 10 years, experienced a conversion from hypothyroidism to hyperthyroidism after receiving the Gam-COVID-Vac (Sputnik V) vaccine. After the first dose, her hormonal metabolism altered from hypothyroidism to euthyroidism, and after the second dose, she developed hyperthyroidism with thyrotoxicosis, which persisted for several weeks before reverting to hypothyroidism. The patient's thyroid-stimulating hormone receptor antibody levels remained normal throughout the period, but a significant increase in the peak systolic velocity of the superior thyroid arteries was observed after hyperthyroidism reverted to hypothyroidism. The autonomic nervous system (ANS) is likely involved in the conversion, and diagnostic assessment of ANS condition should be performed before vaccination in patients with chronic thyroid disorders.
