Sheridan Community Hospital's Community Health Needs Assessment (CHNA) identified five significant health needs: Healthy Lifestyles, Financial Security/Economic Stability, Mental Health, Substance Use Disorder, and Transportation. The hospital's Implementation Plan (CHIP) will address Healthy Lifestyles, Substance Use Disorder, and Financial Security/Economic Stability from April 2023 to March 2025.

For Healthy Lifestyles, the plan includes annual wellness health fairs, free blood pressure screenings, community garden beds, food resources, and community education. For Substance Use Disorder, the plan includes monitoring prescriptions, reporting opioid findings, offering information on Medication Assisted Treatment (MAT), and providing staff education.

The plan aims to increase participation in health fairs and screenings by 10% each year, increase food donations by 10%, and increase MAT participants by 10%. The hospital will also establish networking with community resources and provide education to all staff. Mental Health and Transportation needs will be addressed by other hospitals, organizations, and community partners due to limited resources.
