Summary

COVID-19 infection increases the risk of hyperglycaemia with ketones, Diabetic KetoAcidosis (DKA), and Hyperosmolar Hyperglycaemic State (HHS) in people with or without previously recognised diabetes. Key actions include checking blood glucose in all patients on admission, checking ketones in those with diabetes or high blood glucose, and stopping SGLT-2 inhibitors. Patients with COVID-19 may require adjusted fluid replacement regimens and early involvement of diabetes specialist teams. Insulin resistance is common in ICU settings, and IV insulin protocols may need amending. Alternative subcutaneous regimens may be necessary if infusion pumps are not available.
