Summary

Complex karyotype, older age, and reduced first-line dose intensity determine poor survival in core binding factor acute myeloid leukemia patients with long-term follow-up. A retrospective study of 192 patients from 11 Italian institutions found that patients with inv(16) chromosomal rearrangements had superior disease-free survival and a trend toward longer overall survival compared to those with t(8;21) rearrangements. A complex karyotype strongly impacted survival, while the KIT D816 mutation predicted worse prognosis only in patients with t(8;21). Intensive first-line therapy, including 3-drug induction regimens and consolidation with high-dose cytarabine or stem cell transplant, resulted in better outcomes. Multivariate analysis identified age, severe thrombocytopenia, and elevated lactate dehydrogenase levels as independent predictors of longer overall survival.
