Summary

Septic arthritis is an orthopaedic emergency requiring prompt identification and treatment, as it can lead to joint destruction within days. A 23-year-old immunocompetent lady presented with severe left knee pain and swelling, initially diagnosed with acute gout and treated with NSAID. However, her symptoms worsened, and she was later diagnosed with septic arthritis and necrotizing fasciitis. Synovial fluid from the knee joint aspiration showed growth of Group A Streptococcus (GAS), and a culture from the left calf showed mixed growth. She underwent surgical debridement and intravenous antibiotics, leading to full recovery after 45 days. The case highlights the challenge of diagnosing septic arthritis and necrotizing fasciitis in immunocompetent adults without underlying risk factors, emphasizing the importance of timely management to reduce complications and long-term joint damage.
