Summary

Immunoglobulin gene sequence analysis in chronic lymphocytic leukemia (CLL) has been pivotal in understanding the disease's natural history, with studies from the 1990s revealing the role of immunoglobulin receptors and antigen stimulation. The presence and load of somatic hypermutation (SHM) within the rearranged IG heavy variable (IGHV) genes dichotomize CLL into two categories: unmutated (U-CLL) and mutated (M-CLL). U-CLL is associated with adverse prognostic genomic aberrations, increased B-cell receptor signaling capacity, and an overall inferior outcome. The European Research Initiative on CLL (ERIC) has been at the forefront of immunogenetic research, publishing recommendations for determining IGHV gene mutational status and providing an expert online forum for sequence interpretation. ERIC has also developed the ARResT/AssignSubsets bioinformatics tool and established the IG Network to harmonize IG gene sequence analysis. Recent updates to the ERIC recommendations include technical considerations for determining IGHV somatic hypermutation status, such as the use of EDTA as an anticoagulant, purification of B cells, and direct sequencing methodology. The identification of stereotyped B-cell receptor immunoglobulins has also been proposed as a means of superseding the U-CLL versus M-CLL categorization, with certain subsets emerging as prototypes of aggressive disease. The SHM status of IGHV genes has been associated with clinical response to chemoimmunotherapy regimens, making it a predictive test for guiding treatment decisions.
