Summary

Ondansetron und die Krux der Risikokommunikation zu Arzneimitteln bei Schwangeren: Eine 2018 veröffentlichte Studie assoziierte Ondansetron mit einem teratogenen Risiko, speziell für orale Spaltbildungen. Die Europäische Arzneimittel-Agentur (EMA) initiierte daraufhin eine Warnung vor Anwendung in der Schwangerschaft. Die Studie fand eine statistisch signifikante Assoziation zwischen Ondansetron-Anwendung im 1. Trimenon und dem Auftreten von angeborenen oralen Spaltbildungen, jedoch nur bei isolierten Gaumenspalten. Die Gesamtrate der orofazialen Fehlbildungen war erhöht, aber die Autoren schlussfolgern, dass nach Ondansetron-Exposition im 1. Trimenon drei orale Spaltbildungen auf 10.000 pränatal exponierte Kinder zusätzlich auftreten. Die wissenschaftliche Datenlage ist jedoch widersprüchlich, und ein teratogenes Risiko speziell für orale Spaltbildungen erscheint bei kritischer Betrachtung der Studie nicht bestätigt. Ondansetron kann daher weiterhin "off label" als Mittel der Reserve bei Versagen der für die Schwangerschaftsübelkeit etablierten Antiemetika angesehen werden.
