The Medication First Approach for Opioid Use Disorder treatment is based on 4 principles:
1. People with OUD receive pharmacotherapy as quickly as possible;
2. Maintenance pharmacotherapy is delivered without arbitrary tapering or time limits;
3. Individualized psychosocial services are continually offered but not required;
4. Pharmacotherapy is discontinued only if it worsens the person's condition.
This approach prioritizes providing medical treatment without conditioning it on other service requirements,
similar to the Housing First approach to homelessness, and is consistent with the Substance Abuse and Mental Health Services Administration's definition of recovery.
Maintenance pharmacotherapy with buprenorphine and methadone can reduce fatal opioid overdose rates by 50-70%,
reduce illicit drug use, and increase treatment retention.
A list of 10 Do's and Don'ts for medical treatment of OUD is provided, including not initiating taper or discontinuation in response to client infractions,
not mandating participation in counseling, and individualizing dose decisions based on client factors.
