Summary

Malignant peripheral nerve sheath tumors (MPNSTs) are rare, with a general population incidence of 0.001%, but 8-13% in neurofibromatosis type 1 (NF1) patients. A 23-year-old NF1 patient presented with a 3-month history of throat pain and dyspnea, and a 4-month history of neck mass under the right ear. MRI revealed a right para-pharyngeal mass, initially diagnosed as a neurofibroma in 2008, which had increased in size over time. Biopsy results showed high-grade spindle cell sarcoma, and the patient was diagnosed with multiple cervical lymph node metastases. The patient underwent concurrent chemoradiotherapy, but follow-up showed no regression or progression of the disease. This case report highlights the rarity of MPNSTs in the para-pharyngeal space and the challenges of managing this aggressive tumor.
