Summary

Screening for cervical cancer is shifting from cytology/Pap tests to primary high-risk Human Papillomavirus (HPV) testing, with support options for positive HPV results. HPV testing has higher sensitivity (96.1% vs 53.0%) but slightly lower specificity (90.7 vs 96.3%) than cytology for detecting grade 2 or higher cervical intraepithelial neoplasia (CIN-2+). The World Health Organization recommends HPV DNA tests as a primary detection method starting at age 30, with periodic tests every 5 to 10 years. There are currently three acceptable screening strategies: Pap, Co-testing, and primary HR-HPV testing. The American Cancer Society and US Preventive Services Task Force provide guidelines for screening, including HPV testing alone every 5 years starting at age 25, Co-testing every 5 years, or Pap alone every 3 years. Approved HPV tests include the Cobas HPV test and BD Onclarity HPV test, which offer options for immediate triage for women with positive HPV results. Dual Staining (DS) has higher sensitivity and specificity than HPV-16/18 genotyping or cytology for triage. The transition to primary HPV testing presents challenges, but evidence supports its better sensitivity, and options for triage or HPV testing support are improving.
