Summary

Supracondylar humerus fractures are common in pediatric populations, especially in 0-3 and 4-7 year olds. Treatment is guided by fracture stability and displacement, with closed reduction and percutaneous pinning (CR/PP) being the preferred method. However, open reduction with percutaneous pinning (OR/PP) may be necessary if closed reduction fails or vascular compromise occurs. A study of 204 patients found that age, neurological symptoms, and high-energy injury etiology were associated with the need for OR/PP. A "U-shaped" fracture pattern was associated with neurological symptoms, but not with the need for open reduction. The main indication for OR/PP was failure to achieve a satisfactory CR, and orthopaedic surgeons should have a low threshold to transition to an open procedure in patients with these risk factors.
