Summary

Subscapularis tenotomy is considered an optimal approach to shoulder arthroplasty, as strong repair of the subscapularis is crucial for stability, motion, and strength. Historically, subscapularis integrity was overlooked, but recent reports have highlighted its importance. Lesser tuberosity osteotomy was proposed to minimize subscapularis repair failure, but it has challenges such as complexity and non-union. Tenotomy, on the other hand, is a simple, reproducible, and time-efficient method. Biomechanical studies have compared tenotomy and osteotomy, showing no significant difference in maximum load to failure, stiffness, and elongation amplitude. A prospective randomized trial also found no significant difference in strength, WOOS, and ASES scores between the two techniques, suggesting that neither has a clear advantage over the other.
