- Requester marks the applicable box for the desired form of access: printed copy, transcription, flash drive/CD, or cloud storage.
- Manner of Access:
- Requester marks the applicable box for the preferred manner of access: personal inspection, postal services, courier, fax, email, or cloud share/file transfer.
- Preferred Language:
- Requester specifies the preferred language for access.
- Particulars of Right to be Exercised or Protected:
- Requester indicates which right is to be exercised/protected and explains why the record is required for that purpose.
- Fees:
- Requester acknowledges fee requirements (request fee, access fee, exemption reasons) and preferred manner of correspondence for notification.
- Signatures:
- Requester and Information Officer sign the form.
- For Official Use:
- Information Officer records reference number, date received, access fees, and deposit (if any).
