Summary

On January 14, 2020, Foundation Medicine, Inc. (FMI) discovered unauthorized access to one of its employee's email accounts due to a phishing incident on December 17, 2019 and January 14, 2020. The employee's email may have contained certain protected health information (PHI) of patients, including your full name, date of birth, age, test ordered, ordering physician, and a FMI-assigned ID number. No financial information or Social Security Number was involved. FMI recommends monitoring account statements for any unusual activity. They have taken steps to secure the account, changed the employee's password, and are implementing additional security measures to prevent future incidents.
