Summary

The concept of supervision in nurse anesthetist practice is often misunderstood, with different positions held by the American Association of Nurse Anesthetists (AANA) and the American Society of Anesthesiologists (ASA). AANA views supervision as a continuum, taking into account the nurse anesthetist's education, experience, and capabilities, as well as institutional rules and patient needs. In contrast, ASA's guidelines require anesthesiologists to be heavily involved in patient evaluation, anesthesia planning, and administration. However, historical evidence and court rulings suggest that the level of supervision required for reimbursement purposes is not the same as that needed for state licensing requirements or ethical practice. The Joint Commission on the Accreditation of Health Care Organizations (JCAHO) has adopted standards that are more flexible than ASA's requirements, allowing for supervision by a licensed independent practitioner who may not have anesthesia privileges. Ultimately, the dispute between AANA and ASA has been ongoing, with courts rarely reviewing the issue due to the wide latitude given to healthcare providers.
