By signing, participants acknowledge and agree to the following:

- Risk Awareness: They understand that participating in these events involves significant risks of injury, disability, death, loss, or damage to person or property, including potential permanent paralysis or death.
- Assumption of Risk: Participants knowingly and freely assume all such risks, both known and unknown, even those arising from the negligence of organizers, officers, officials, directors, agents, employees, other participants, sponsoring agencies, sponsors, advertisers, premises owners, and lessors.
- Release and Indemnification: They release and hold harmless all "Releasees" (as defined below) from any liability for injury, disability, death, loss, or damage to person or property caused by their negligence or intentional misconduct, whether arising from the Releasees' negligence or otherwise.
- Compliance with Rules: Participants agree to comply with event rules, participant release of liability, and safety regulations. Boat operators and equipment operators are solely responsible for safe operation and the safety of all occupants.
- Insurance: Participants using a boat during the event must have or obtain property damage, excess medical, watercraft liability, and/or contractor's insurance with reasonable limits, covering any injury or damage incurred in connection with the event.
- Youth Safety: If participating in a youth event, participants have been provided with training compliant with the Youth Safe Sports Act of 2017.
- Waiver of Privacy Rights: Participants waive their rights of privacy and publicity regarding the use of their name, voice, likeness, video, audio, and fishing tips by organizers and their affiliates without compensation.
- Application Rejection: Organizers reserve the right to reject any application for any reason.

Signatures:

- Angler(s): List all participants with their signatures, dates, and ages.
- Boat Captain (if applicable): Signatures, dates, and age.
- Parents/Guardians of Minority Age Participants: Consent to the release and agree to indemnify Releasees for any incidents related to their child's participation. Include parent/guardian signatures, emergency contact numbers, and dates.
