Phosphine primarily affects the cardiovascular and respiratory systems, causing symptoms such as restlessness, irritability, drowsiness, tremors, cough, dyspnea, and vomiting. Acute exposure can lead to cardiac arrest, peripheral vascular collapse, and pulmonary edema, with most deaths occurring within 12-24 hours after exposure.
