Summary

The OHDSI CDM is becoming the standard for representing EHR data for research, with projects like the N3C and All of Us Research Program adopting it. However, the restrictive nature of these enclaves has made it difficult to use OHDSI tooling like ATLAS. A manual method was developed to export and run OHDSI-generated cohorts in the N3C enclave, involving importing JSON cohort definitions into ATLAS, manually exporting to SPARK SQL dialect, and processing with a Python script. The transformed cohort definition was run against the entire N3C enclave, including 19.6 million individuals and 26 billion rows, with a total time of 30 minutes to create the workbook and 36 minutes for query execution. The goal was to determine the feasibility of translating cohort definitions into a format that can be run in the N3C secure enclave, allowing non-technical users to develop clinically meaningful cohorts in Atlas and export them to a format that a programmer can work with.
