1. 在宅医療、ACPと医療倫理との関係：在宅医療を中心に実践したACPは、患者の意思を尊重するルールを実践するものである。
2. あるべきACPの実践を難しくする事情と在宅医療の理念：COVID-19罹患者は感染症の急性患者であり、短期間で死に至るリスクを秘めていること、短期間で治療方針を決定することは困難であること、重症患者数に対する人工呼吸器の数が圧倒的に少ない事態になれば患者の意思が必ずしも治療方針に反映されない可能性があることなどがACPの実践を難しくする。
3. 提案：できるだけ早くケアのゴールや高度な治療についての誠実な話し合いを始めるべきである。多職種で在宅医療を支える専門職は連携してACPのプロセスを支えるべきである。家族や親しい人が関わることがふさわしい。伝え続け、声に耳を傾ける。患者と家族のかけはしとして家族の声を患者に、患者的声音を家族に届ける。
4. 留意点：話し合いは押し付けにならないようにする。選択は急がず、受け手と担い手の負担に注意する。
5. 在宅医療を巡って提起される患者の意思・ACPをめぐる倫理的問題：在宅で療養中の認知症の人がCOVID-19罹患者となり、家族が在宅療養を希望した場合などは、患者の意思を尊重することが重要である。
