AHPA Guidance Policy for labeling of dietary supplements containing fungi dietary ingredients:

Fungal dietary ingredients must be identified on the label by common or usual name, part or parts of the fungal ingredient present, and in order of predominance by weight.
Parts of fungi ingredients may include fruitbody, mycelium, sclerotium, spores, etc.
Ingredients other than dietary ingredients must be disclosed in the ingredient list preceded by "Other ingredients".
The word "mushroom" is not required on the label, but if used, it must be modified to identify the part(s) of the fungal dietary ingredient(s) contained in the product.
Specific terms such as "mushroom mycelia and fruitbodies" or general terms such as "mushroom complex" or "mushroom composite" may be used, but the specific fungi and/or fungi parts present must be disclosed in order of predominance by weight.
A glossary of terms relevant to fungal ingredients and dietary supplement labels is provided, including definitions for hypha, fruitbody, mushroom, mycelium, primordium, sclerotium, spore, substrate, dietary ingredient, dietary supplement, label, labeling, marc, and principal display panel.
