Summary

A substantial majority of Part D plans use specialty tiers, with 76% of PDPs and 90% of MA-PDs using them in 2008. Most plans employ 25% or 33% coinsurance for specialty tiers. About 1 in 5 drugs are placed on a specialty tier by at least one plan, but there is little consensus among plans about which drugs belong on specialty tiers. Specialty drugs face utilization management restrictions in over 1/3 of plans and are more likely to be subject to prior authorization. Brand-only and injectible drugs are more likely to be placed on specialty tiers, with 4 classes (antineoplastics, immunologics, antivirals, and antibacterials) accounting for nearly 2/3 of specialty drugs.
