The Personal Information Protection Law, which was established in May 2003 and revised in 2017, is currently being reviewed every three years due to changes in social conditions. Our association properly obtains, uses, and stores personal information of students provided by each school at the time of membership or when receiving insurance benefits, in accordance with relevant laws and regulations, and thoroughly implements safety management for personal information protection. We request the following responses from all school-related personnel:
1. When seeking comprehensive consent for providing personal information to related organizations in the "Entrance Guide" etc., please include "Fukuoka Prefectural School Safety Promotion Association" in the list of related organizations.
2. In cases other than 1, please respond in a format suitable for each school on the page seeking agreement for personal information provision, regarding information provision to "Fukuoka Prefectural School Safety Promotion Association".
