Summary

Suicide attempters among depressed older patients exhibited more severe past and present depressive symptoms, higher current suicidal ideation, earlier age of onset, and greater lifetime depressive episodes compared to nonattempters. Baseline white-matter lesions (WML) were significantly higher in suicide attempters, and their WML growth increased more over time than in nonattempters. While WML predicted cognitive decline in older depressed patients, a history of suicide attempt was unrelated to baseline cognitive functioning or rates of cognitive decline. The study confirms that depression severity and WML are associated with suicide attempts in geriatric depressed patients, indicating that suicide attempts predict neurologic changes like WML progression. These neurobiological alterations may contribute to poorer long-term outcomes in older suicide attempters despite no direct cognitive impairment difference between groups.
