Summary

A 2-year-old boy with Down's syndrome presented with projectile vomiting since starting complementary feeding, and abdominal distension. Initial tests, including abdominal ultrasound, opaque enema, and manometry with rectal biopsy, were normal. Oesophagogastroduodenal transit showed partial obstruction of the second portion of the duodenum with malrotation, which was surgically corrected. However, symptoms recurred 15 months later, revealing a duodenal web in the third portion. The case highlights the diagnostic challenge of combined extrinsic and intrinsic duodenal obstruction in children with Down's syndrome, requiring a high index of suspicion and early treatment.
