Summary

A 54-year-old patient with left hyperchronic pleural empyema was treated with open window thoracostomy (Eloesser window) after multiple failed drainages and antibiotic treatments. The surgical procedure involved creating a window in the thoracic cavity, evacuating and washing the empyema cavity, and suturing the edges of the flaps to the parietal pleura. Postoperative follow-up showed significant improvement in the patient's general state, reduction of secretions, and granulation of the cavity walls. The patient was discharged after 3 weeks and is currently being followed up for window toileting, with an anticipated evolution towards quasi-total occlusion of the thoracic window in 9-10 months.
