Summary

A 48-year-old male with hereditary spherocytosis presented with a 3.2-cm posterior mediastinal mass, which was suspected to be a neurogenic tumor. However, further examination suggested extramedullary hematopoiesis (EMH), and a technetium-99m phytate bone marrow scan showed asymmetric increased uptake. The patient underwent video thoracoscopic surgery, and the mass was completely excised, confirming the diagnosis of EMH. The patient had no complications and was discharged on the 5th postoperative day. EMH is a rare disease that can occur in patients with hematologic disorders, and it should be differentiated from neurogenic tumors and other posterior mediastinal diseases. Surgical excision is recommended in patients with symptomatic masses or suspected malignancy.
